The report you will need in eighteen months
A collision on a ski slope, a fight at the bar, a burn on the restaurant line, a fall from a roof on a solar install. Different industries, one shape: what happened, who was in it, what you did about it, and at what time. Business Dash records all four in the same place.
Anybody can file. Not everybody can read.
Filing an incident needs no permission at all, and that is deliberate: an org where reporting an injury requires a manager is an org where injuries go unreported until one of them is serious. Reading the register, deciding money and referring a matter to HR are separate grants held by the people whose job that is.
An injury is not a disciplinary record
A worker who was hurt is never chargeable — not for any kind of incident, not ever. And an injury on its own can never become an HR matter: referring one requires somebody to write down what the person actually did, and a sentence that only restates the injury is refused. A business whose safety reports feed its disciplinary file is a business that stops hearing about near misses.
Undecided is not the same as nothing
A charge left blank means a human has not decided yet. A write-off records a considered zero, with a name against it. Six months later the difference between forgiven and forgotten is the entire conversation, and only one of those two records can have it.
